Have you taken him to a vet to have a complete physical? When you go for yearly vaccinations, the vet will do a brief physical exam, but having a seperate appt for just the exam will allow more time to check your dog. The vet might want to do some bloodwork to see if there might be an underlying physical cause of discomfort for the dog.
I know you mentioned taking the dog to classes as well as having private instruction. Was this obedience training? That is often a step in the right direction when dealing w/behavior issues.
One technique that I've heard used w/dog behavior is the "Nothing in life is free" regime. The dog has to "earn" everything from food to the right to sit on comfy cushions...everything must be earned by exhibiting PROPER behavior to the owner. Perhaps looking into this method might help.
There are also new drugs on the market to help take the edge off some animal's dispositions. You might want to ask the vet if your dog is a candidate.
