After each treatment she will improve, only to begin to suffer again within a month. She eats a chicken-rice based food. She is an indoor crate-trained dog without kennel bedding.

Michele, is she a droopy-eared dog? For instance, like a lab, with ears that hang down? Yeast need warmth and dark for an environment to grow in (and the right pH), and droopy-eared dogs provide that environment with their ear-set. My lab had chronic yeast problems for a while and I went through several courses of panalog and antifungals with the vet. The panalog didn't work as well as I'd hoped and I asked if I could use Neosporin instead. He agreed and okayed me using hydrogen peroxide to clean her ears and neosporin to coat the insides with. Check with your vet about using such a course for cleaning (like every day for a while) and see what he/she says. If it is okayed I will go through how I did it with you.
