For his breath - chloraphyll or charcoal treats help with cleaning his breath. Does he let you handle his mouth? Give pills? Try buying a fingerstall toothbrush (PetCo or PetSmart should have them) and a flavored toothpaste for dogs (don't use human toothpaste, please) in a flavor he is mad for - liver, maybe? - and try just doing a couple of teeth at a time. Then a break of at least an hour, then another two teeth. Or do maybe three teeth per day. Gentle voice and perhaps a hard treat to make the process easier should help.
